Confident adult smiling after full-arch dental implant restoration
01 — All-on-4 Implants

All-on-4 implants in Downey.

Full-arch tooth replacement using just four dental implants, a permanent, fixed alternative to traditional dentures.

02 — What Are All-on-4 Implants

A new smile in a single procedure.

All-on-4 is a full-arch dental implant treatment that replaces all the teeth in your upper or lower jaw using just four strategically placed implants. The implants support a fixed (non-removable) bridge of replacement teeth that look and function like natural teeth. The result is a permanent, stable smile that does not slip, click, or require nightly removal.

All-on-4 is designed specifically for patients missing all or most of their teeth in an arch, or whose remaining teeth cannot be saved due to advanced gum disease, severe decay, or other conditions. It is also an excellent alternative for patients currently wearing traditional dentures who are tired of the limitations and discomfort that come with removable replacements.

Dr. Ben Reyhani, the practice's implant specialist, leads every All-on-4 case at our Downey office. Dr. Stephen Kallaos, our in-house periodontist, supports complex cases requiring bone grafting or specialty foundation work. Having both implant and periodontal specialists on staff allows us to handle most All-on-4 cases without referrals to outside offices.

Most patients receive a temporary set of teeth on the same day as implant placement (often called teeth in a day), with permanent restorations placed 4-6 months later after full healing. The investment is significant, but the result is a permanent, life-changing smile that typically lasts 25+ years with proper care.

03 — Why All-on-4

Why All-on-4 changes everything.

Compared to traditional dentures, All-on-4 offers significant advantages that affect daily quality of life.

  • Fixed, never removed

Unlike dentures, All-on-4 teeth are permanently attached to the implants. They never come out for cleaning, never slip while talking, and never require adhesives or special care routines.

  • Eat what you want

With All-on-4, patients can eat the foods they have given up on with dentures, steaks, apples, corn on the cob, hard breads. Chewing function returns to nearly natural levels.

  • Preserves jaw bone

Implants stimulate the jaw bone the way natural tooth roots do, preventing the bone loss that occurs when teeth are missing or replaced with dentures alone.

  • Looks completely natural

All-on-4 teeth are custom-designed to match facial structure, lip support, and aesthetic goals. Most people cannot tell that the teeth are not natural.

  • Long-term durability

Properly placed implants typically last 25+ years, often a lifetime. The replacement teeth (prosthesis) may need refurbishing every 10-15 years, but the implants themselves rarely need replacement.

  • Better than individual implants

Replacing every missing tooth with an individual implant would require 10-12 implants per arch and significantly higher cost. All-on-4 achieves comparable results with just four implants per arch, far more efficient.

04 — Is It Right For You

Who is a candidate for All-on-4.

All-on-4 is designed for patients with specific situations. Here is when it typically makes sense.

  • Missing all or most teeth in your upper or lower jaw
  • Remaining teeth cannot be saved due to advanced gum disease, severe decay, or fractures
  • Currently wearing traditional dentures and dissatisfied with fit, function, or comfort
  • Adequate bone density to support implant placement (or candidate for bone grafting)
  • In good general health and able to undergo a multi-hour procedure with sedation
  • Committed to long-term maintenance including routine cleanings and annual implant evaluations
  • Financially prepared for the investment in permanent tooth replacement

Not every patient is a candidate.

Patients with severely insufficient bone, uncontrolled diabetes, recent radiation therapy in the jaw area, certain medications affecting bone metabolism, or significant medical conditions may not be candidates. Heavy smokers face higher implant failure rates and are typically counseled to quit before treatment. We evaluate each case carefully.

05 — The Process

What to expect from All-on-4 treatment.

Most cases follow this six-stage process from initial consultation to final restoration.

  1. 01

    Consultation & 3D Imaging

Free consultation with Dr. Ben Reyhani, the practice's implant specialist. 3D cone beam imaging captures detailed views of your jaw, bone density, and existing dental anatomy. We discuss treatment goals, timeline, and cost.

  2. 02

    Treatment Planning

Detailed treatment plan including implant positioning, prosthetic design, and any preliminary procedures (extractions, bone grafting, or sinus lifts when needed). Plan is finalized with you before scheduling.

  3. 03

    Surgical Day

In a single appointment (typically 4-6 hours), any remaining teeth are extracted, the four implants are placed, and a temporary prosthesis is attached the same day. You leave with teeth, often called teeth in a day.

  4. 04

    Healing Period

Over the next 4-6 months, the implants integrate with the jaw bone (osseointegration). You wear the temporary prosthesis during this period. Soft food diet for the first weeks, gradually returning to normal eating.

  5. 05

    Final Prosthesis Design

Once healing is complete, impressions are taken to design the permanent prosthesis. Material choice (acrylic, zirconia, or hybrid) is based on aesthetic priorities, durability needs, and cost considerations.

  6. 06

    Final Placement

The temporary prosthesis is removed and the permanent prosthesis is attached to the implants. The transformation is permanent, you walk out with the final result.

06 — Upper Vs. Lower

Differences between upper and lower All-on-4.

Upper Arch (Maxilla)

Upper jaw considerations

  • Often requires sinus lift or bone grafting due to thinner bone naturally present in the upper jaw
  • Cosmetic visibility is higher, lip support and smile aesthetics are key planning factors
  • Implant angulation is critical for avoiding the sinus cavity and nasal floor
  • Typically takes slightly longer to plan and execute than lower arch

Lower Arch (Mandible)

Lower jaw considerations

  • Generally has denser bone, often allowing direct implant placement without grafting
  • Functional priorities (chewing strength) often outweigh aesthetic priorities
  • Faster healing and earlier loading possible in many cases
  • Often less complex surgically than upper arch
07 — Common Questions

Questions about All-on-4?

All-on-4 is a major investment in your smile and quality of life. Honest answers to the questions patients ask most often.

Ask a Question

All-on-4 represents a significant investment, typically ranging from $20,000-$30,000 per arch depending on materials, complexity, and any preliminary procedures needed (extractions, bone grafting, sinus lifts). Both arches together are typically less than double a single arch due to shared planning and procedural efficiency. We provide a clear cost breakdown at your consultation. CareCredit financing is available, often allowing patients to spread payments over multiple years.
