Gingivitis (Early Gum Disease)
Inflammation of the gums caused by plaque buildup. Reversible with professional treatment and improved home care. Symptoms include bleeding gums, redness, and tenderness during brushing.
Specialized treatment for healthy gums and the foundation of every smile, provided in-house by Dr. Stephen.
Periodontics is the dental specialty focused on the gums, supporting bone, and connective tissue around your teeth, the structures that hold your smile in place. Periodontal disease is one of the most common chronic conditions in adults, and it is the leading cause of tooth loss in the United States.
Most general dental practices identify periodontal issues during routine cleanings but refer treatment out to a separate specialist office. We do not. Dr. Stephen Kallaos, our in-house periodontist, treats gum disease and performs periodontal procedures at the same Downey office where the rest of your dental care happens.
Dr. Stephen handles the full range of periodontal care: early gum disease (gingivitis), advanced periodontal disease (periodontitis), gum recession, periodontal surgery, soft tissue grafting, crown lengthening, and dental implant support. Most patients can complete their entire treatment plan in our practice without referrals to outside offices.
Untreated periodontal disease does not just affect your mouth. Research links advanced gum disease to heart disease, diabetes, stroke, and other systemic health issues. Treating it early matters, and having a periodontist on staff makes early treatment significantly easier to access.
From early gum issues to advanced periodontal disease, here are the conditions we treat most often.
Inflammation of the gums caused by plaque buildup. Reversible with professional treatment and improved home care. Symptoms include bleeding gums, redness, and tenderness during brushing.
Untreated gingivitis that has progressed to bone loss around the teeth. Requires specialty treatment to halt progression. Symptoms include gum recession, loose teeth, persistent bad breath, and bite changes.
Gums pulling back from teeth, exposing roots and creating sensitivity. Causes range from aggressive brushing to genetic factors to periodontal disease. Soft tissue grafting can restore lost tissue.
Spaces between teeth and gums where bacteria accumulate. Deeper than 4mm typically requires professional treatment. We measure pocket depth at every periodontal exam.
Loss of jawbone supporting teeth, often resulting from advanced periodontal disease. Bone grafting can rebuild lost bone, especially when planning dental implants.
Bone augmentation, soft tissue management, and site preparation for upcoming dental implants. Dr. Stephen works closely with Dr. Kiro Farag on complex implant cases requiring specialty foundation work.
Periodontal treatment ranges from non-surgical deep cleanings to advanced surgical procedures, matched to the severity of your case.
Non-surgical deep cleaning that removes plaque and tartar from below the gumline and smooths root surfaces to prevent reattachment. Often the first line of treatment for early to moderate periodontal disease, sometimes performed in two visits with local anesthesia.
Surgical procedure that reduces pocket depth around teeth, allowing better cleaning and slowing or halting disease progression. Performed in our Downey office under appropriate sedation when needed.
Restoration of lost or receded gum tissue using grafts that can come from your own palate or donor sources. Treats gum recession, protects exposed roots, and improves smile aesthetics.
Reshaping of gum and bone tissue to expose more of a tooth. Used both for restorative purposes (so a crown can be properly placed) and for cosmetic improvement (correcting a 'gummy smile').
Many patients do not realize they have periodontal disease until it is advanced. Here are the warning signs that a periodontal evaluation is worth scheduling.
If any of these resonate, schedule a consultation. Most periodontal conditions are treatable when caught early, and many can be reversed entirely. Waiting only makes treatment harder, longer, and more expensive.
Schedule with Dr. StephenMost periodontal cases follow this five-stage process from evaluation to maintenance.
Free consultation with Dr. Stephen. He examines your gums, measures pocket depths, takes any needed X-rays, and identifies the specific periodontal conditions present. You leave with a clear understanding of your case.
Dr. Stephen designs a treatment plan based on your specific diagnosis, ranging from improved home care and routine periodontal cleanings for mild cases to scaling and root planing or surgical intervention for advanced cases.
The actual treatment phase, which depends on your case. May involve one or more deep cleaning visits, surgical procedures performed in our office, soft tissue grafting, or other targeted treatments. Often completed in a few weeks.
After active treatment, your gums need 4 to 8 weeks to heal. Dr. Stephen re-evaluates the affected areas and confirms whether disease progression has been halted. Most cases see significant improvement in pocket depth and tissue health.
Patients with treated periodontal disease typically transition to periodontal maintenance cleanings every 3 to 4 months rather than the standard 6-month schedule. This ongoing care is essential to prevent recurrence.
Periodontal disease is common but rarely discussed. Here are the questions Dr. Stephen hears most often from patients.
Ask a QuestionCommon signs include bleeding gums when you brush or floss, gum recession, persistent bad breath, and tender or red gums. Many patients have early gum disease without obvious symptoms, however, which is why routine dental visits matter. We measure pocket depths at cleanings and can identify gum disease in its earliest, most treatable stage. Patients with risk factors like diabetes, smoking history, or family history of gum disease should be especially proactive about evaluation.
Early gum disease (gingivitis) is fully reversible with professional treatment and improved home care, often within a few weeks. More advanced periodontal disease (periodontitis) is not fully reversible because the supporting bone has been lost, but it can be effectively treated and stabilized. The goal in advanced cases is to halt progression, preserve teeth, and prevent further bone loss. Acting early matters significantly.
Treatment depends on severity. Mild cases respond to scaling (deep cleaning), improved home care, and routine maintenance cleanings every 3 to 4 months. Moderate cases typically require scaling and root planing under local anesthesia. Advanced cases may need periodontal surgery, soft tissue grafting, or bone grafting. Dr. Stephen recommends the least invasive treatment that will effectively address your specific case.
Most periodontal treatments are performed under local anesthesia, so the procedure itself is comfortable. Some mild soreness is normal for a few days after deep cleaning or surgery, manageable with over-the-counter pain relievers. Patients describe the discomfort as similar to having a tooth filled. We use sedation options for patients with significant anxiety or for more extensive surgical cases.
Most dental insurance plans cover scaling and root planing, periodontal maintenance, and many surgical procedures, sometimes at higher reimbursement rates than routine preventive care because periodontal disease is classified as a medical condition. Coverage varies by plan. We verify your specific benefits before treatment begins so you have a clear understanding of what your insurance covers and your expected out-of-pocket cost.
Regular cleanings (prophylaxis) are preventive care for healthy gums and clean above and around the gumline. Periodontal cleanings, including scaling and root planing, are therapeutic treatments that clean below the gumline to remove tartar and bacteria from periodontal pockets. Patients with active or treated periodontal disease typically need periodontal maintenance cleanings every 3 to 4 months rather than standard 6-month cleanings, with deeper cleaning techniques to control disease.
Yes. Research has linked advanced periodontal disease to heart disease, stroke, diabetes complications, respiratory disease, and even some pregnancy complications. The bacteria associated with gum disease can enter the bloodstream and trigger systemic inflammation. Treating gum disease is not just about preserving teeth, it is part of overall health management, especially for patients with diabetes or cardiovascular conditions.
Most general dental practices identify gum issues during cleanings but refer you to a separate periodontist office for treatment, often weeks away. Having Dr. Stephen on staff means your treatment can begin sooner, your records and care are coordinated with the rest of your dental team, and you do not have to drive across town for follow-up appointments. For complex cases involving implants, restorative work, or multiple specialties, this integration significantly improves outcomes and reduces total treatment time.
Dr. Stephen works alongside our general dentists, not in a separate office across town. That integration matters for periodontal patients.
Dr. Stephen Kallaos completed three years of specialty training in periodontics beyond dental school and is a Diplomate of the American Board of Periodontology. Periodontal care is his clinical focus, not a side service.
Dr. Stephen works from the same patient records as our general dentists. When periodontal issues come up during a cleaning, treatment starts within the same practice.
Dr. Stephen provides specialty consultations and treatment planning in both English and Spanish. Periodontal decisions are easier to make when communication is clear.
Dr. Stephen and Dr. Kiro Farag work together on complex implant cases requiring bone grafting or soft tissue management. The right specialist on each part of your treatment plan.
Permanent tooth replacement led by Dr. Kiro Farag, often coordinated with Dr. Stephen for bone grafting and soft tissue support.
